The below forms are required by the School of Chemistry and Biochemistry. Guidelines for writing thesis and thesis approval forms are available from the Georgia Tech Graduate Office.Be sure to consult the Chemistry Graduate student handbook and Institute deadlines.
Doctoral Forms:

Research Advisor Selection (due by Feb. 15 of first year)

PhD Thesis Committee Selection (due by end of summer semester of first year)

Evaluation of PhD candidacy exam (due upon completion of PhD candidacy exam at end of spring semester of second year)
- Request for Admission to PhD candidacy (due upon completion of PhD candidacy exam at end of spring semester of second year)
- Approval of Final Research Review (due 6 months prior to graduation)
- Petition for Degree (see Institute deadlines)
- Evaluation of Thesis Defense (due upon completion of thesis defense)
- Certificate of Thesis Approval (due upon completion of thesis defense)
- Exit Interview Forms
- Institute Enrollment Waiver (if applicable)
Master of Science in Chemistry Forms:
- Research Advisor Selection (Thesis Plan only)
- MS Thesis Committee Selection (Thesis Plan only)
- Request for Approval of MS Thesis Topic (Thesis Plan only)
- Petition for Degree (Thesis or Nonthesis plans)
- Approved Program of Study (Thesis or Nonthesis plans, available from Tech Tower)
- Certificate of Thesis Approval (Thesis Plan Only)
- Exit Interview Forms
- Institute Enrollment Waiver (Thesis or Nonthesis Plan, if applicable)
