REPORTING ACCIDENTS (First Aid Response and Transportation Policy Procedure)
In the event of an emergency call GT Campus Police 4-2500 or 404-894-2500.
- Guidance for the procedures for reporting accidents and reporting worker's compensation are located here.
- Chemistry Accident / Incident Report Sheet [PDF] Click on the link to access the accident form. The form is setup to type directly in to a pdf file template.
- Worker's Compensation Claim must be reported by the supervisor of the injured person. Click on the link for the procedure and information that will be required. The claim number will be used for medical, hospital, rehabilitation, etc., insurance payments.
