Near-miss ReportA Near Miss is an unplanned event that did not result in injury, illness, or minor damage – but had the potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality or damage; in other words, a miss that was nonetheless very near. History has shown repeatedly that most loss producing events(incidents), both serious and catastrophic, were preceded by warnings or near miss incidents. Recognizing and reporting near miss incidents can significantly improve worker safety and promote an organization’s safety culture.

Accident/Incident ReportIt is mandatory to complete a report after an accident/incident. In addition, GT Environment Health and Safety (EHS) required that injury form be completed. The report will not only provide accurate documentation of the event, but it will also serve as a learning tool for others.
Laboratory Weekly and Monthly Self-Inspection ChecklistLaboratory self inspection are required on a weekly and a monthly basis. The completed inspection sheets must be dated, signed, and filed in the green safety binders. Download the checklist here.
Quarterly Laboratory Self-InspectionLaboratory self-inspections are required by the School of Chemistry and Biochemistry on a quarterly basis.

Unattended OperationsAll unattended experiments must be accurately labeled with information essential to personnel not familiar with the experiments, such as emergency responders. This information must enable people to easily assess the hazards associated with the experiments, as well as provide the contact information of the people who can provide additional information if needed.
